Health Home Services

The Health Home program is a service available to eligible Washington Apple Health (Medicaid) members with chronic conditions.

The program was created under the ACA to provide care coordination and services to high risk Apple Health members. These services specifically assist members with serious chronic conditions, like diabetes, asthma, heart disease, etc.

Health Home services include:

• Care management
• Care coordination and health education
• Transitional care
• Individual and family support services
• Referrals to community and social support services
